    • Collects, maintains, and makes available to authorized users timely, accurate, secure and complete patient health information
    • Controls the release of medical information from patient medical records in accordance with applicable standards
    • Provides authorized individuals with timely access to patient information without compromising the security of patient records
    • Reviews returned patient records for completeness
    • Ensures the presence of provider signatures/initials
    • Ensures each sheet of patient information contains appropriate identifying information
    • Ensures minimum necessary protected health information is used only for purposes identified or as required by law and not further disclosed without patient authorization
    • Ensures that the amount of information contained in patient records is manageable and relevant
    • Adheres to established timelines to ensure availability of patient information
    • Ensures completeness of record before forwarding to Provider for appointment
    • Processes all Affiliated Contractor (AC)/Medicare Administrative Contractor (MAC) inquiries within ten (10) business days of receipt, including entering information into the Advanced Case Tracking System (ACTS), checking the Fraud Investigation Database (FID), closing the ACTS entry and reporting findings back to the AC
    • Enters all investigations into the ACTS and the FID
    • Maintains the JOA tracking tool for revisions to all JOAs controlled by the task order
    • Types and maintains various workgroup meeting minutes
    • Oversees training evaluation distribution, collection and scoring for QMB reporting
    • Serves as the key control point for records/file management
    • Retrieves records from the file room when requested
    • Documents new box numbers into Iron Mountain database working closely with staff for retrieval of stored material and coordinating with the storage facility vendor
    • Orders and sends records to offsite storage location
    • Ensures that Medicare Part A and B prepay claims received will be counted and logged and appropriate screen prints made for entering into the Prepay Database
    • Scans all Medical Review records and other documents as needed
    • Performs responsibilities associated with RAC Warehouse Database as needed
    • Provides desk coverage for front desk receptionist on an as needed basis
    • Provides clerical and administrative support for the task order business units as required according to individual unit workload requirements and needs
    • Performs other duties as assigned by the Operations Manager and Medical Records Manager
    • Processes initial medical record requests, including: printing pull lists, requesting letters and coversheets, photocopying of materials, scanning mailing labels, packing initial request envelopes with all required documents, and mailing via postage meter and U.S. Postal Service
    • Opens and correctly routes all mail relating to the distribution and workload processes
    • Performs the initial screening of records received from providers and AC’s, and forwards all screened rejects to the Customer Service Representative
    • Scans records into temporary storage utilitizing FIFO (first in, first out) order, and allocates space for incoming modules by stocking and rearranging storage bins
    • Distributes records from temporary storage to reviewers, distributes repaired records, and responds to reviewer requests for records
    • Retrieves records from reviewers
    • Returns completed records to permanent storage and technical stop records to the Customer Service Representative
    • Scans records into permanent storage and maintains storage bins
    • Retrieves records from temporary or permanent storage for managers, clinical supervisors, report lists, and as directed by the Medical Records Manager
    • Correctly fulfills the assigned workload to the MRS
    • Assists with IQC processes or other tasks as assigned
    • Maintains a neat, organized work area
    • Attends required education and training programs
    • Complies with departmental policies and procedures



    • High Schoold Diploma or equivalent related experience

    • At least six (6) months of medical record experience
